and check out materials at any of our locations. Fill out this simple application
online, then stop by any San
Francisco Public Library to receive and activate your card. Electronic
applications will be retained for one month.
If you have had a San Francisco Public Library card during the past four years, DO
NOT FILL OUT THIS FORM. Please visit any San Francisco Public Library with a photo ID
and proof of residential address if you need a replacement card.
