Description Offers a multidisciplinary assessment and case management program for adults 18 years of age or older with functional impairments either physical or mental. Assists frail elderly, stroke and heart attack survivors, and disabled people to maintain maximum independence and avoid nursing home placement. Provides nursing and professional care, rehabilitation including physical, occupational, and speech therapy. Provides daily hot lunches and snacks. Provides social workers and a dietitian for consultation. Provides wheelchair accessible transportation to and from the facility for people living in the vicinity. Sponsors mutual support groups for caregivers of individuals with dementia including Alzheimer's Disease. Groups are free, call for additional information.

Eligibility San Francisco resident; 18 years of age or older; frail, under a physician's care and unable to maintain current level of functioning.
Fee Based on ability to pay, $30 to $70 per day. Medi-Cal and private pay accepted.
