| Description |
Provides licenses to health care facilities in San Francisco, San Mateo and Northern Santa Clara counties under the specifications of the California Administrative Code (CAC), Title 22, Division 5. Health facilities include nursing homes, clinics and home health agencies. Provides licenses to hospitals in San Francisco and San Mateo counties. Provides information on facilities and regulations upon request. |

There is a public file of deficiency reports that can be reviewed in person between the hours of 9:30 a.m. to 11:30 a.m. and 1:30 p.m. to 4:30 p.m., Monday through Friday. Public are welcome to review the public file. Accepts complaints regarding all types of facilities at the toll-free number, 800-554-0353. Call for more information. |
