| Description |
Provides intensive short-term case management, primary and behavioral health care, transportation, support and advocacy for the team’s chronically homeless clients. SF HOT’s main goal is to help the chronically homeless obtain permanent housing. |
| Eligibility |
Clients, with rare exception based on extenuating circumstances, are chronically homeless individuals who have agreed to work with the team on a voluntary basis, and who were encountered through outreach efforts or through referrals from specific agencies (including SFPL Main Library) which have a pre-determined working relationship with SF HOT. |
| Fee |
No cost to clients. |
| Transportation |
SF HOT arranges for transportation of its clients, as needed, to its offices. Clients are also met wherever they are staying, including the street, hospitals, stabilization rooms, jail, rehabilitation centers, etc. |
