| Description |
Provides emergency home visits to seniors including psychiatric assessment, case management, information and referral, evaluation for possible hospitalization or half-way house admission, and initiation of conservatorship. Outpatient services include assessment, home visits, outreach, information and referral, medication, and consultation. |
