Description Health at Home is the home health agency of the Community Health Network, San Francisco Department of Public Health. The agency serves an average of approximately 800 clients per year. 25 percent are individuals with HIV disease, 25 percent have life threatening illnesses such as cancer or end-stage heart or respiratory disease that requires palliative care. The remaining 50 percent of clients are the frail elderly, those with chronic and disabling illness and others recovering from an acute episode or trauma. Services include: nursing care, social work, home care aide and rehabilitation therapy, as well as volunteer, bereavement and spiritual support. The agency also seeks volunteers to assist administrative office support staff.
Eligibility Eligible individuals are usually homebound and have a need for skilled nursing, physical or speech therapy, and live in San Francisco. Managed care clients and those served through a special contract may have other eligibility criteria (i.e., those served through CARE Title 1 (Ryan White CARE contracts).
