| Description |
Provides services for chronically ill individuals and their families. Services include individual, couple and family counseling; support groups; case management; benefits counseling; information and referral; meal delivery; practical and emotional support volunteers; spiritual support; educational programs; and emergency financial assistance. Also offers attendant care through the Help-at-Home program; volunteer opportunities are available with this program. |

Under 60 years of age with chronic illness or disability; and their partners and families. Jewish for case management or financial aid. |
| Fee |
Varies for counseling and therapy services. Case management is on a sliding scale from free to $100 based on income. Medicare and private insurance accepted. |
