Description A multi-service agency offering a broad range of programs to seniors and functionally impaired adults including health care, senior center and social services, employment, training and job placement, and housing. All program staff is bicultural and bilingual. Muni Fast Passes are also available at the agency. Operates senior centers in several locations throughout the city. Services include a hot noon meal; recreational and educational activities; and information and referral. Center hours are Monday through Friday from 9 a.m. to 1 or 2 p.m. See separate listings under Self-Help for the Elderly in this database or call 415-677-7600 for information on additional locations. Provides a home-delivered meal to homebound seniors Monday through Friday. Also provides transportation to and from agency senior centers. Seniors may also schedule transportation to and from medical appointments. Provides social services including case management, counseling, and assistance with completing or translating documents at Senior Centrals and other locations throughout San Francisco. Recreational and educational activities available at the Centers may include: consumer, health and nutrition classes; exercise programs; bingo; singing; outings; arts and crafts; tai chi; karaoke and social dancing. Offers housing information and assistance. Provides information about and referral to low-income housing and information/education about tenants' rights and other consumer issues. Staff also mediates landlord/tenant disputes. Services are available at 407 Sansome Street. Offers a citizenship program. Services include helping clients to complete citizenship application forms, ESL and citizenship preparation classes, tutoring and naturalization preparation. Services are available at 407 Sansome Street and 27 locations throughout the Bay Area. The Northeast Senior Central Program also located at 407 Sansome Street (415-438-4800) provides information and referral and assists clients to secure needed services.
Offers home care services including skilled nursing, physical, occupational and speech therapies, medical social services and certified home health aide services. Hospice services are provided to patients who have been diagnosed with a terminal illness and have a life expectancy of 6 months or less. Services include skilled nursing, medical social services, counseling, certified home health aide services, and pastoral care. Offers employment training and job placement for housekeepers and home health aides including occupational English as a Second Language (ESL) training for seniors. Arranges for a housekeeper or home health aide to provide in-home services through the Housekeeping Home Aide Registry. Emergency in-home supportive services provides temporary housekeeping and personal care services to seniors who reside in San Francisco. The Senior Community Services Employment Program places older workers in paid positions in community agencies located in San Francisco. The Technology and Information Empowerment Center, 601 Jackson Street (415-989-3535) provides Internet access and bilingual computer training. The Jackie Chan Adult Services Center, located at 408 22nd Avenue (415-677-7556) houses two programs, an Alzheimer's Day Care Resource Center and an Adult Day Health Care Program. Both programs offer day-long programs with nursing, medical social services, physical, occupational and speech therapies and activities. Programs operate Monday through Friday and provide transportation to and from the center. Operates the Lady Shaw Senior Center a 70-unit low-income apartment complex located at 1483 Mason Street; Leonard House a 15-bed residential care home located at 1771 Waller Street; Autumn Glow Alzheimer's Care Home, 654 Grove Street, a 15-bed facility for seniors with Alzheimer's Disease; and Sunrise Center a 34-bed residential care home located in Santa Clara.
