| Description |
Provides culturally sensitive, comprehensive and on-going nurse and social work case management to clients with disabling status of HIV/AIDS. Certified Nurse Attendant or Homemaker Support is also available based on RN Case Manager assessment. Clients must reside in San Francisco, and have a PMD that signs off as Medical Provider.. |

No income restriction for case management. 400% NPL for homecare.. |
